CONFIDENTIALITY: All information disclosed within sessions and the written records pertaining to those sessions are confidential and may not be revealed to anyone without your written permission, except where disclosure is required by law.

 

WHEN DISCLOSURE IS REQUIRED BY LAW: Some of the circumstances where disclosure is required by the law are: where there is a reasonable suspicion of child, dependent or elder abuse or neglect; where a client presents a danger to self, to others, to property, or is gravely disabled.

 

EMERGENCIES: If there is an emergency during our work together, or in the future, after termination, where your practitioner becomes concerned about your personal safety, the possibility of you injuring someone else, your practitioner will do whatever she can within the limits of the law, to prevent you from injuring yourself or others and to ensure that you receive the proper care. For this purpose, she may also contact the person whose name you have provided as an emergency contact.

 

CONFIDENTIALITY OF EMAIL AND TEXT COMMUNICATION: Potential risks of using electronic communication may include, but are not limited to; inadvertent sending of an e-mail or text containing confidential information to the wrong recipient, theft or loss of the computer, laptop or mobile device storing confidential information, and interception by an unauthorized third party through an unsecured network. Please do not use e-mail or texts for emergencies.

 

TELEHEALTH: The electronic and online systems used during our sessions incorporate security protocols designed to protect the confidentiality of client information and to safeguard the data against intentional or unintentional corruption.Although these safeguards are in place, the potential risk of data breach and/or electronic corruption exists. Your personal information is stored on a HIPAA compliant server. In the event that the online portal experiences technical problems (either via the practitioner’s computer, the client’s computer or the website hosting the video) every attempt will be made to reconnect. If that does not resolve the problem, a second online portal will be used and you will be notified via email of how to connect to the new session. If the technical issues are due to computer failure or power failure, the session will continue via telephone.

 

HOLISTIC INTEGRATIVE SERVICES: Healing sessions promote harmony and balance within, helping to relieve stress and supporting the body's natural ability to heal. Many of the mind-body techniques that may be utilized during sessions seek to reduce unresolved stress. Healthy Holistics does not make claims as to what may have happened in the past. During a session, all memory events are considered “emotional reality” (i.e. perception), because these events may or may not correspond with actual or historical reality.DISCLAIMER: Stress reduction techniques and tools are each widely recognized as a valuable and effective complement to conventional medical care. However, techniques used are not a substitute for psychological or psychiatric therapy, chiropractic care, or medical services. PAYMENTS: Clients are expected to pay for sessions in full unless other arrangements have been made.

 

CANCELLATION: Since scheduling of an appointment involves the reservation of time specifically for you, a minimum of 48 hours’ notice is required for rescheduling or canceling an appointment. Unless we reach a different agreement, the full fee will be charged for sessions missed without such notification.

 

HOLD HARMLESS: I hereby release, waive, discharge and covenant not to sue Healthy Holistics and/or Laurie Holland for any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the releasees, or otherwise, while participating in a coaching session, or while in, on or upon the premises where a coaching session is being conducted, while in transit to or from the premises,

or in any place or places connected with the coaching session.
